Allocated hours include assessment, analysis, report writing and a feedback session for the individual.

The process

  • Talk our clinical team first if you’re unsure
  • Meet with the person to discuss the reasons for the referral.
  • Agree on the desired outcomes of the assessment
  • Fill in the formal referral form and ask them to sign the consent section
  • Return the completed form to us at [email protected]
  • We allocate a practitioner to complete the assessment
  • The practitioner shares the content of the report with the person before it is finalised and returned to us
  • We password-protect the final report and send it through to the referring manager, to discuss with the person and agree next steps
  • You can talk through the recommendations with us to identify how best to support the person through the next steps, if that would be helpful.
